By checking this box, upon termination either voluntary or involunatary, I authorize Wm. Sharpe Farms to deduct the cost of the clothing purchased during my first year of employment. Deduction amounts will be dependent on the time of service:  100% up to the first 6 months of employment, 50% between 6-12 months of employment. After 1 year of service Sharpe Farm Supplies will absorb the cost for the branded wear.

By checking this box I authorize Wm. Sharpe Farms to deduct the cost of the items to be replaced, if any items are missing or have been

damaged through my negligence.
